As part of providing a psychological service to you, Dr Segal will need to collect and record personal information from you that is relevant to your current situation. This information will be a necessary part of the psychological assessment and treatment that is conducted.
Purpose of collecting and holding information
The information is gathered as part of the assessment, diagnosis and treatment of the client’s condition, and is seen only by the psychologist. The information is retained in order to document what happens during sessions, and enables the psychologist to provide a relevant, continuous and informed psychological service.
Confidentiality
All personal information gathered by Dr Segal will remain confidential and secure except where:
- It is subpoenaed by a court.
- Failure to disclose the information would place you or another person at serious and imminent risk.
- Your prior approval has been obtained to provide a written report to another professional or agency (eg. lawyer or insurance company).
- Your prior approval has been obtained to discuss the material with another person, eg. parent, partner or employer.
- If disclosure is otherwise required or authorised by law.
Correspondence with referring doctor and GP
In line with Medicare referral requirements, Dr Segal will correspond with your referring psychiatrist and/or GP regarding your treatment.
Fees
Medicare will cover 10 sessions per year under a Mental Health Care Plan (MHCP) from your GP or a referral from your psychiatrist. The rebate from Medicare is $141.85. Your initial MHCP referral will cover 6 sessions. Should you require further sessions, you will need to obtain a MHCP review referral for a further 4 sessions. If have exhausted the 10 Medicare funded sessions, you may be able to claim a rebate from your Private Health Insurance.
If you choose not to use Medicare, or to claim from a Private Health Fund you do not require a referral.
Payment of your account in full is required on the day of service. We will process the Medicare rebate for you.
Attending each scheduled appointment is important for therapy continuity and the therapeutic process.
Cancellation Policy
If you have to cancel a session, please provide a minimum of 24 hours’ notice. This allows time to accommodate waitlisted clients.
A full cancellation fee is charged for less than 24 hours’ notice or missed appointments (non-rebatable).
